Both the Signed Client Services Agreement & Release and Waiver Form and Signed Agreement Form for Guidelines should be attached in the upload sections of the FHMA Online Referral Form. Please include the First and Last Name of the Client in the file name.

NEW: We have created online agreement and guidelines forms that can be accessed and signed on your celllphone.

Once your online referral form and Shopify order is submitted - please send an email to FHMA Furnishing Hope at operationsmanager@furnishinghopema.org to ensure we receive your client’s materials.
